**Gratis rings are provided by USASF for all active rostered athletes and two roster coaches. Any additional athletes or coaches have the option to purchase a ring at www.teamjewelry.com.**

All names and orders will be verified against your USASF team roster before processing.

Gym/Studio Information – fill out each space.
Shipping Address – all rings will be shipped together to this address.
Ring Size Information – for warranty purposes, we need the full name, email address and phone number for EVERY ring ordered.
Finger Size – available ring sizes are 4 – 14 increasing by half sizes.
Ring Style (Band Size) – two sizes available. Small band (thinner) or large band (thicker).

Please allow 8 – 10 weeks for delivery.
